The team consists of two Occupational Therapists, Clinical Care Coordinator, two Social Prescribers and Care Coordinator we cover six GP Surgeries, Ash Surgery, Minster Surgery, Westgate Surgery, Birchington Medical Centre, Broadstairs Medical Practice and St Peters Surgery. Our aim is to prevent unnecessary hospital admission and to promote health and wellbeing for our patients and their families/carers. We also aim to empower and support patients to take responsibility for their own health and wellbeing. We complete holistic and comprehensive assessments and have very good links with many other services in the community to ensure we are meeting all the needs of the patients we see. We also aim to reduce GP appointments, reduce the need for GP time and reduce frequent calls to the surgery thus reducing the surgery workload.

At The Romney Marsh Community Hub, we wish to help the health and wellbeing over the over 50’s. We support people in trying new experiences, meeting new people and have access to a range of services to keep people engaged and active within the local and surrounding communities. Whether people want to meet friends for a coffee or bite to eat, enjoy a hobby or interest with like-minded people or navigate your way through benefits and allowances, we are here to help. We have a range of different activities and friendly classes that they can try such as Pilates, Tai Chi, Yoga, Keep Fit or even our seated exercise class. We run a number of craft classes for those that wish to learn new skills or continue practicing skills they have. Our day service provides a place where older people can meet with others of a similar age group for friendship and social interaction. We can also provide transport to and from a client’s home to help. Our information and advice services are here to help and support people. Benefit checks may reveal they are entitled to more than what they thought, we can help with the paperwork and or help submit forms online for them if they struggle with this. Our information and advice is impartial, easy to digest and is offered freely. We offer a selection on home cooked meals prepared daily in our kitchen. People are welcome to enjoy these meals here with friends, or if preferred we can deliver direct to the customers door to have at home.

Via Refer Kent, KCF is looking to connect with professionals in the county who support individuals and families. KCF has three funds that can offer grants to individuals and families. Our grants focus on funding: - respite activities for young carers - respite activities for adult carers taking care of another adult and - equipment for young people with disabilities, long-term illnesses and/or life limiting conditions, where the equipment benefits their mental and physical health. General criteria for our grants: the individual must live in Kent or Medway, be able to demonstrate financial disadvantage and items / services cannot be for something that local/national authorities have a statutory duty to provide. We help Carers to make a life of their own outside caring by providing quality care services offering peace of mind while they enjoy some time to themselves. We off a Short Breaks Service, which is an opportunity for Carers to step away from their caring role for a few hours each week, to enable them to recharge their batteries, get some sleep, or carry out the essential tasks of everyday living, whilst we also offer a pre-bookable replacement care to enable Carers to attend planned health appointments, and the treatments they need in order to stay well. We provide a Carer and Dementia Crisis Service which offers free, practical support, designed to prevent the breakdown of a caring situation that might otherwise lead to a hospital or care home admission. We also offer End of Life Care, which supports Carers at some of the most challenging times in their lives. We have a Dementia Outreach service with 1-1 support to families living with dementia. Our volunteer service helps carers and the people they care for by providing free, practical, emotional and companionship support, as well as expert advice, information and guidance.
