Via Refer Kent, KCF is looking to connect with professionals in the county who support individuals and families. KCF has three funds that can offer grants to individuals and families. Our grants focus on funding: - respite activities for young carers - respite activities for adult carers taking care of another adult and - equipment for young people with disabilities, long-term illnesses and/or life limiting conditions, where the equipment benefits their mental and physical health. General criteria for our grants: the individual must live in Kent or Medway, be able to demonstrate financial disadvantage and items / services cannot be for something that local/national authorities have a statutory duty to provide. If you contact KCF via Refer Kent, about an individual or family you think could benefit from one of our grants, you will still have to complete our nomination form. Paddock Wood Community Advice Centre (“PWCAC”) provides a free, confidential, and impartial advice service for the residents of Paddock Wood and surrounding rural parishes. We offer face-to-face appointments as well as a drop-in service. We provide advice about a wide range of matters, including consumer issues, debt and money, employment, family matters (children or relationship breakdown including domestic abuse), housing and homelessness, mental health support and welfare benefits (including representation at tribunals). PWCAC holds the Advice Quality Standard (AQS) mark.

We compete holistic assessments within the patient/client home environment. This is done through an informal conversation with the person(s) requiring support. We can support with benefit maximisation, advocacy or even support within the services offered through Age UK. We can also support clients/patients to access services within the wider sector, this could be care agencies, MDT, District Nurses, Kent fire and rescue safe and well being, our team will support with as much or as little as they would like. Age UK is open 5 days per week , however outside of these hours there will be an option to leave a message or an email facility.

The service is available to all East Kent residents aged over 55 years, regardless of whether or not they are receiving any other services from Adult Social Care. It is also available to those under the age of 55 years, who meet the eligibility criteria and have complex needs.

We help Carers to make a life of their own outside caring by providing quality care services offering peace of mind while they enjoy some time to themselves. Some of our Services: • Short Breaks Service: offers adult Carers a chance to step away from their caring role for a few hours each week, to enable them to recharge their batteries, get some sleep, or carry out the essential tasks of everyday living, whilst we also offer a pre-bookable replacement care to enable Carers to attend planned health appointments, and the treatments they need in order to stay well. • Carer and Dementia Crisis Service: offers free, practical support, designed to prevent the breakdown of a caring situation that might otherwise lead to a hospital or care home admission. • End of Life care supports Carers at some of the most challenging times in their lives. • Dementia Outreach service with 1-1 support to families living with dementia. Our volunteer service helps carers and the people they care for by providing free, practical, emotional and companionship support, as well as expert advice, information and guidance.
