Description
You Matter Community Project CIC is a new Medway-based initiative dedicated to supporting residents aged 60+ who are navigating post-hospital recovery or facing social isolation.
At You Matter Community Project CIC, our mission is to improve the well-being of our elderly residents by reducing social isolation through targeted support, health promotion and empowering pathways to community participation.
We are currently accepting clients needing our support and recruiting enthusiastic volunteers who are interested in giving back to their communities or gaining hands-on experience while supporting a charitable cause.
To ensure we provide the highest quality of support to those most in need, clients must meet the following criteria:
• Age: Must be aged 60 or older.
• Location: Must be a resident within the Medway, Kent area (covering Rochester, Strood, Chatham, Gillingham, and Rainham).
• Support Need: Must be experiencing (or at risk of) social isolation, or currently recovering from a hospital stay.
• Mobility: Must be able to transfer in and out of a vehicle independently or with minimal assistance (as our volunteers are not trained for manual handling/lifting).
Scope of Service (What we do)
Zero-Cost Support: All services are free for users (Non-clinical / personal care)
Check-in Support: Non-clinical companionship to help build confidence after returning home from the hospital.
Well-being Pathways: Targeted support to reduce isolation and promote community participation.
Free Transport: Door-to-door escorted travel to GP surgeries, dental appointments, and hospital outpatient clinics.
Social Signposting: Connecting individuals to local community groups, lunch clubs, or hobby circles.
Free Transport: To access our Saturday social club, where we provide: Free refreshment, entertainment, games, art and craft, knitting, basic IT support with access to tablets/ iPads, free SIM and data (by Goodthings Foundation)
Free Referral: Signposting to further support
Exclusion Criteria (What we do NOT provide)
To ensure the safety of our clients and volunteers, we cannot accept referrals for:
• Personal Care: We do not assist with bathing, dressing, toileting, or feeding.
• Clinical/Medical Care: We do not administer medication, change dressings, or provide medical monitoring.
• Emergency Transport: We are not an ambulance service; we are for pre-booked routine appointments only.
• Advanced Dementia/High-Risk Behavioural Needs: Where a client requires specialist medical supervision during transit or 1-to-1 clinical monitoring.
